The case team records intake and formulation. An owner or manager needs approval to join the case; administrative access alone does not open its notes.

Write and sign session notes, record risk and enter outcome totals from the centre's forms. Corrections stay alongside the signed original.

Confidential case files
- A case file opens only for the people on that case. No role or permission opens it on its own, not even the owner's: a manager asks to join, and someone already on the case approves or declines.
- Every opening, print and document download is logged and shown on the case, with who added or removed whom, every request and every handover.
- Reception books and bills without being told what the therapy is: the calendar, client lists, invoices and receipts say "Therapy session" and "Therapist", never counselling or psychotherapy.
- When a therapist leaves, switching off their login takes them off every case, and the case is handed over to a new lead with the reason on record.
Consent, notes, risk & outcomes
- Record consent to therapy and to record keeping before the intake: from the client, or from a parent or guardian for anyone under 18. Nothing new is written on a case without it.
- Intake, formulation and plan, session notes, reviews and a closing summary. Signed notes are never edited: a correction is added beside the original.
- Risk is graded at every intake and review. A high grade, or a PHQ-9 self-harm answer above zero, stays on the case and on the case team's Today screen until the risk is reviewed.
- PHQ-9, GAD-7 and CORE-10 totals with their standard severity bands, and a distress trend across the sessions. Between-session tasks print as the client's handout; every other record prints marked confidential.
Calendar, packages & billing
- A day or week calendar with a column per therapist, weekly series, rooms and a waiting list. The Today screen shows each therapist's sessions and notes to sign.
- Single sessions or session packages with their own balances and expiry, dues, payments by cash, card or mobile banking, and the centre's cash book and profit and loss.

Who can open a case file?+
Only the people on the case: the lead therapist and anyone they or the team add. An owner or manager can ask to join, but cannot open it until someone on the case approves. Who opened, printed or added whom, and when, is shown on the case.
What does reception see?+
What booking and billing need: the client's name, times and dues. The calendar, lists, invoices and receipts say "Therapy session" — never the notes, the scores or the kind of therapy.
Is this for psychiatrists?+
It is for counsellors and psychotherapists, and has no prescription. A psychiatrist who prescribes uses our Prescription module; a centre with both simply has both.
Are the questionnaires answered on screen?+
No. The therapist scores the centre's own form and enters the total; the software shows its severity band. The PHQ-9 self-harm question is recorded on its own.
